The Perinatal High Risk Management/Infant Services System (PHRM/ISS) is a multidisciplinary case management program. It was established to help improve access to health care and to provide enhanced services to certain Medicaid-eligible pregnant/postpartum women and infants. The enhanced services for this target population includes case management, psychosocial and nutritional counseling/assessments, home visits and health education.
Who Is Eligible
Medicaid beneficiaries are eligible to participate in this program when a physician, nurse practitioner or certified nurse mid-wife Identifies one or more positive risk factors on the PHRM/ISS Perinatal Screening Form. This is a voluntary program. It is not mandatory for a beneficiary to participate in this program.
A positive risk factor is any chronic condition that may cause a pregnant woman or her fetus to become ill during or after the pregnancy, as determined by a prenatal assessment.

If you're eligible and decide to take part in the PHRM/ISS program, our team of nurses, nutritionists and social workers will help you with:
- Finding a doctor for your pregnancy and for your baby
- Medical referrals
- Delivery planning
- Health information and diet advice
- Family planning
- Home visits
