These forms are provided for those who prefer to complete course forms manually rather than using the web-based system. All of these forms are in the Adobe Acrobat PDF format. The free Adobe Reader may be downloaded by following the link below, near the bottom of the page. Some of these forms are designed to be completed by typing your response directly into the form. Unless you have the full version of Adobe Acrobat (not the Adobe Reader), you will ONLY be able to print these forms upon completion. YOU WILL NOT BE ABLE TO SAVE YOUR DATA.
- Course Request and Instructor Verification Form – This form must be submitted by the course coordinator prior to the start of the class.
- Course Roster – This form should be completed for each participant in the class. This will be the only roster needed for refresher blocks and driving courses. EMT courses should submit this form after the first class meeting. Use a separate page for each student and enter as much information as possible. The instructor should indicate the status of the student and sign the form.
- EMT-Basic Course Completion Roster – This form is to be submitted upon the completion of an EMT – Basic course. This form verifies that the minimum requirements of the program have been met and that all documentation is on file. The College Registrar must sign the form.
